Birth Mom Vacation Acceptance Agreement

I. Agreement *

It is my understanding that the following, and only the following, will be provided to the recipient upon acceptance of a Funding Love Birth Mom Vacation: - Transportation and/or airfare to and from Walt Disney World Resort in Orlando, Florida. - A Walt Disney World vacation package that includes a 4-night stay at a Walt Disney World Resort, a 2-day park hopper ticket, and the Disney Dining Plan. - Trip-planning assistance from an Authorized Disney Travel Professional.

II. Terms *

I agree that all terms upon which I received this Funding Love Birth Mom Vacation are true. I agree that I possess legal documentation that I have placed my biological child(ren) with an adoptive family.

III. Termination *

I agree that either party shall have the right, forthwith and without further notice, to terminate this Agreement by written notice to the other, upon the occurrence of a breach in Terms of Agreements listed above. Upon termination, I claim no rights to funds, goods, or services allocated towards this vacation booked under my name or otherwise.
